Medication at School

Sometimes it is necessary for children to take medication during the school day. Children who take prescription medication MUST have a physician’s authorization form completed by the physician and the parent consent form completed. Both forms must be submitted to the office before medication can be dispensed. The forms are available in the school office. Non-prescription medication requires the completion of the parent consent form also available at the school office. Only medication that is required to be given 3 or more times a day, to be taken with meals or at a specifically scheduled time within the school day need to be administered at school. All medications must be sent in the original pharmacy labeled container and with the students grade and room number. Medications are to be given to the office by an adult (students are not allowed to transport medication).
